As ADCES advances its strategic plan, the Centers for Disease Control and Prevention is advancing health equity through two large cooperative agreements, (CDC-2321 MATCH) and (CDC-2320 DELTA). ADCES is an active participant in this work at the national and state levels. In this session, attendees learn how to get involved in advancing evidence-based strategies to improve the quality of care, increase access to care, and prevent diabetes complications through community partnerships within their states, counties, and organizations. Some examples of the strategies include expanding Medicaid coverage for diabetes prevention and engaging community health workers as diabetes care team members.
