Health programming that can significantly impact the health of minority populations must enhance trust in the health systems where the care is provided and should lead to sustainably encouraging healthy lifestyle changes. This session describes how Cities Changing Diabetes Houston worked with community members to co-create the Faith and Diabetes Initiative that offers DSMES in houses of faith, which have very high levels of trust in their communities.
