In the realm of diabetes care, change is the only constant. Throughout the person's lifespan with diabetes, there will be continual requirements for the care team to reassess treatment. Various factors, whether permanent or temporary, often prompt a need for modifications to medication therapy. These prompting circumstances can include changes in glycemia status, development of complicating factors, transitions of care, sick day management, perioperative considerations, switching between medications, and travel across time zones. This presentation aims to focus on how diabetes care and education specialists can identify opportunities and provide guidance for de-prescribing and making transitional medication adjustments.
