Technology is reshaping the way Diabetes Care and Education Specialists (DCES) view glucose values, measure diabetes outcomes, and collaborate with people with diabetes to obtain optimal health. This session will explore a process improvement initiative to increase rates of data sharing in a diverse urban pediatric center. In practice, the DCES faces many barriers to data sharing, such as social determinants of health, educational gaps, issues with user buy-in, and ongoing access to technology. Presenters discuss strategies for implementing interventions to increase the sharing of CGM data to mitigate acute complications and improve time-in-range.
