Despite the availability of effective treatment options and diabetes medication management guidelines, less than 51% of persons with diabetes achieve an A1C of less than 7%. Therapeutic inertia is impacted by the person with diabetes, the clinical team, and various health system factors. This session will outline how the use of a medication algorithm, shared decision-making, team communication, and identification of barriers to self-care by the DCES can be used to halt therapeutic inertia and improve health outcomes across a diverse population.
