Health inequities are driving increases in the number of people living with diabetes risk throughout the US, disproportionately impacting low-income and minority populations. Strategies to address these trends are demonstrated through expanded public payer coverage of diabetes prevention and management programs. By operationalizing CDC-recognized Umbrella Hub Arrangements (UHAs), a centralized administrative infrastructure increases access to Medicare and Medicaid reimbursements uniting multiple sectors: healthcare, community-based organizations, payers, and public health. In this session, participants will learn about the multi-faceted aspects of UHAs that address access for priority populations, SDOH health screening, and health-related social needs.
