Quality improvement to achieve optimal diabetes care and education services is essential and should occur on an ongoing basis. To remain competitive, healthcare systems must continuously change. Change management is the art of making diabetes services function well and continue to excel. Such change is informed by evidence-based practice to translate research findings into health care practices and decision-making. Diabetes care and education specialists are key drivers-as change agents-to continuously improve diabetes care and education services. This session will guide attendees through the change management process and provide them with examples and resources to successfully lead change.
