People with diabetes have an increased risk for complex medication treatment plans, readmissions, and increased costs associated with hospital admission and subsequent discharge. Supporting people with diabetes during this transition of care requires a health system level, interprofessional, and interdepartmental approach. This session highlights opportunities and examples of improving processes to support the education and clinical management of people with diabetes during and following hospital discharge. Approaches discussed will include operational, information technology, educational, and clinical interventions.
