The University of Nebraska Medical Center’s Diabetes on Track project is a grant-funded program from the Diabetes Care Foundation to transform diabetes care and education in rural Nebraska. In this session, presenters will illustrate their progress in improving prediabetes and diabetes care in two rural communities. They will provide outcomes data from each intervention including EHR dashboards and reports, a health system and provider incentive program, an RN health coach and CDCES mentorship, and an added CGM venture.
