M01B - Illustrate the Effectiveness of a Pharmacist-Run CGM Program in an FQHC

Continuous glucose monitoring has proven effective in aiding individuals with diabetes to achieve glycemic targets. However, data is lacking in the medically underserved patient population. This session reviews a study assessing the impact on glycemic management through the introduction of a pharmacist-led CGM service in a federally qualified health center. The program was initiated to enhance quality measures, with a specific focus on improving HbA1c levels. Participants in the program received CGM support and were closely monitored by a clinical pharmacist for 3 months, during which medication adjustments and lifestyle interventions were tailored based on the CGM results.
