Persons with diabetes are twice as likely to be hospitalized for heart failure compared to those without diabetes. Heart failure is associated with significant morbidity, mortality, and financial burden. Inpatient diabetes care and education specialists are well-positioned to provide education and support for hospitalized patients with heart failure to improve self-care and to reduce readmission risk. During this presentation, attendees will learn about evidence-based strategies to reduce readmissions that can be implemented by the inpatient DCES. An example of an inpatient DCES-led educational intervention will be shared.
