The Centers for Medicare and Medicaid Services (CMS) Medicare Diabetes Prevention Program (MDPP) is an evidence-based behavior change intervention for Medicare beneficiaries with prediabetes, using curriculum and standards from the Centers for Disease Control and Prevention (CDC) National Diabetes Prevention Program. CMS and CDC are working together to expand enrollment in MDPP at the national level and address disparities in diabetes in underserved areas. This session will include lessons learned in the first five years of MDPP collaboration, new strategies to address diabetes inequities through outreach, capacity building, and data sharing, and opportunities for practitioners to be involved in MDPP.
