Diabetes is a complex health condition affecting 25% of older adults. Geriatric syndromes occur more often in older adults living with diabetes, placing them at risk for healthy aging inequities that limit their abilities to reach their full health potential. This session will use a case-based approach to guide participants in the principles of a comprehensive geriatric assessment and strategies to intervene on common issues including malnutrition, functional limitations, cognitive impairment, falls, and polypharmacy. Attendees will apply these concepts to provide supportive equitable personalized care that considers the unique social determinants of health in this population.
