Community Health Workers (CHWs) play a critical role in serving our most vulnerable populations. As trusted lay members of the community, CHWs provide culturally appropriate education and information in a way that is easy to understand. CHWs also collect data to inform future outreach. In 2020, the Virginia Department of Health and the Virginia Community Health Worker Association partnered to launch the CHW Toolkit Program. The toolkits provide hands-on education to promote learning and discussion of prediabetes, diabetes, and other chronic health conditions. This session will examine the positive impact this program has had on communities and the CHW workforce.
