To achieve optimal health and quality of life for people with diabetes or prediabetes, diabetes care and education specialists must adopt a strategic health equity approach. In this session, attendees will learn how MATCH (Mobilizing Access Through Capacity Building and Health Equity) advances health equity in diabetes prevention and care by identifying priority populations, understanding root causes of inequities, assessing community readiness, and collaborating to drive positive health outcomes. The MATCH framework can help attendees increase participation and retention of high-risk populations in diabetes care by working alongside community partners to sustain their work in preventing, delaying, or managing diabetes
