Over 25% of US adults have limited or low health literacy. Assessing the literacy needs of hospitalized patients with diabetes is essential for providing appropriate education and fostering diabetes self-care. Unfortunately, gaps exist between some patient’s literacy capabilities and the readily-available teaching materials provided by inpatient clinicians. Identifying and closing these gaps can increase patient engagement in their diabetes management and reduce risks for adverse events after discharge. This session will outline the importance of identifying patient-specific literacy needs, review common educational content available in major healthcare systems, and discuss strategies to optimize teaching for vulnerable patients.
