Continuous glucose monitors (CGM) have demonstrated benefits in improving A1C, decreasing hypoglycemia, and improving self-management skills for people living with diabetes. Disparities in access to CGM exist, particularly in underserved communities. Additionally, primary care providers in underserved community care need support in managing complex diabetes, as patients may utilize community health centers over specialty care. This session outlines 3 implementation methods utilized in an urban FQHC to increase access to CGM for patients and support primary care providers in CGM utilization. Presenters will share several case studies and attendees will learn practical tips for implementation in their practice.
