Families with health disparities living in a lower socioeconomic environment need support and empowerment to make life-long self-care changes. Nutrition care for youth and adolescents with T2D is recommended based on evidence-based guidelines in medical nutrition therapy. In this session, a Registered Dietitian DCES will share examples of best clinical practices to impact the health of this underserved, disadvantaged population as they take on more self-care and enter adulthood.
