S12A - Learn Lessons about CGM Implementation in Primary Care: The PREPARE 4 CGM Study

The role of continuous glucose monitoring continues to expand, not only in type 1 diabetes, which is often managed in primary care settings but especially in type 2 diabetes, which is overwhelmingly (90%) managed in primary care. Though primary care practices can vary greatly, primary care clinicians have expressed clear interest in implementing CGM into practice. However, how best to do so is largely unknown. This session reviews a project to investigate three distinct CGM implementation strategies for primary care practices. Seventy-three primary care practices throughout Colorado enrolled and chose to implement CGM via either a virtual CGM initiation service (virCIS) or the American Academy of Family Physicians TIPS CGM practice implementation suite of education and tools. Practices choosing TIPS were randomized to use the module as designed or to receive professional practice facilitation services to guide implementation. Practice characteristics, implementation progress, and CGMs ordered were measured, and surveys and interviews were also conducted.
