People living with schizophrenia are five times more likely to develop type 2 diabetes. Limited resources along with challenging social determinants of health lead to shorter life expectancy and obstacles for the CDCES in caring for this population. Unconscious bias may impact the care of patients with mental health diagnoses. This session shares a detailed case study showing creative, “outside the box” thinking, and perseverance by the CDCES working with the extended diabetes care team and the community, yielding a positive outcome in a complicated situation where values and culture collide in an urban setting.
