Diabetes affects a disproportionate number of Americans, and successful diabetes management starts with patient and family education. Inpatient stays offer an opportunity to begin or continue the learning process; this is where diabetes care and education specialists play an essential role. This session showcases a project, where a clinical nurse specialist provided diabetes education for patients identified by interdisciplinary care team members and tracked thirty-day readmission rates among these patients. The presenter will share outcomes, potential cost savings, and how the results can be used to provide an impetus for the development of an interdisciplinary inpatient diabetes education team.
