Adult patients with diabetes account for over 30% of noncritically ill hospitalized patients. Hospitalized patients with diabetes are at risk for complications and longer length of stay. The complexity of inpatient glycemic management has led to the emergence of inpatient glycemic services which support a multidisciplinary team approach to glycemic management. This session describes how our hospital designed a glycemic management program based on the Joint Commission's certification requirements for performance improvement as a roadmap to standardize care at the hospital. The program followed the Lean Six Sigma principles to develop subprojects based on identified gaps in care.
