What support do patients with diabetes receive when they leave acute care? Have you ever wondered what struggles and psycho-social issues patients face in their homes? Unnecessary hospitalizations and complications occur every day. The transition from inpatient to home is a challenge for both patients and clinicians. This session will address the important components of an interdisciplinary home care diabetes program. This session addresses how to bridge the gap from acute care to community care, improve continuity, and what institutions can do across the continuum of care.
