Youth in the US are largely not meeting A1C goals. Youth from non-white ethnicities or lower socioeconomic status have even higher A1C’s. These children have less access to high-speed internet to gain access to remote monitoring medical services. This presentation informs attendees about a groundbreaking clinical study designed to test the financial sustainability of CDCES-led virtual diabetes care and improve diabetes-related health and outcomes for pediatric individuals in a rural and underserved community in New York State. Presenters will share initial learnings from billing Medicaid and some commercial plans using remote patient monitoring and principal care management codes.
