P314 - Diabetes Support and Interventions in a Federally Qualified Health Care Center

The low frequency of medical appointments poses a significant barrier to effectively treating diabetes. At best patients are seen every 3 months, leaving the person without clinical monitoring for a relatively long time. This poster showcases a quality improvement effort focused on providing more frequent contact with a diabetes care and education specialist for people with a new diagnosis of type 2 diabetes and anyone with an AIC of over 9%. In addition, medications were adjusted to prevent therapeutic inertia, and a continuous glucose monitor was included as an option.
