P300 - Initiation of Diabetes Reports to Decrease Therapeutic Inertia and Improve Patient Health

As part of the Diabetes On Track project, the role of the Diabetes RN health coach (RN HC) was established in a family medicine clinic in rural Nebraska. Diabetes RN HC implemented reports to identify patients who needed to be screened for prediabetes and patients with pre-existing diabetes who needed updated healthcare metrics. This poster describes ambulatory quality priority metrics and how diabetes education was initiated for patients through collaborative efforts.
