P231 - Powering Personalized, Preventive Care for Native Americans Living with Complex Diabetes

Native Americans have the highest diabetes rates among all racial groups, at 14.5%. Given these grave disparities, a recent survey was conducted to explore Native Americans’ experience living with type 2 diabetes. This poster reviews the results of the 400+ people surveyed on the impact of diabetes on Native American lives. The study aimed to understand the obstacles faced and shed light on how care teams and diabetes care and education specialists can best support this population. The poster highlights tailored guidance for DCESs to drive more personalized, preventive care - including best practice approaches to supporting SDOH challenges.
