The quest for trusted, practical type 2 diabetes prevention and management resources starts here. People with diabetes make countless decisions in their efforts to successfully manage the condition in ways that work for them. Additionally, health care providers face challenges in meeting people where they are, often with limited time and resources. This poster presentation showcases a map highlighting the evidence-based, audience-centered resources from CDC’s Division of Diabetes Translation. From videos, toolkits, guides, and worksheets, find resources to help promote behavior change and deliver tailored programs and services to encourage long-lasting lifestyle change.
