P224 - Addressing Health Equity and Diabetes Care in Alabama

Increased access to healthy foods and diabetes support is pivotal in addressing health equity, but unfortunately, is not readily available in health disparate communities. This poster discusses the unique partnerships created with the American Diabetes Association to address food insecurity and diabetes in Alabama, helping to connect people with diabetes to local resources, including the establishment of a 10-acre community garden. Presenters will review how they successfully implemented a diabetes support program addressing social determinants of health as a part of diabetes care, creating clinical and community linkages that are proven to have the greatest impact on health.
