P223 - FQHC: The Rising Champion for Persons Living with Diabetes in Underserved and Rural Communities

It is hard for persons living with diabetes to find the support and help they need to manage their diabetes. This is the case with many persons living with diabetes who reside in West Virginia. This poster highlights how Federally Qualified Health Center (FQHC) programs help bridge the gap by offering care that provides medications at a reduced cost, sliding scale fees for services for people without health insurance, and dental and mental health services.
