P222 - A Diabetes Self-Management Education and Support Pilot Study in an Underserved Community

Diabetes is the 7th leading cause of death in the United States and is linked to increased risk of some cancers including liver, pancreas, uterine, colon, breast, and bladder cancer. Diabetes Self-Management Education and Support (DSMES) programs are known to improve diabetes self-management knowledge, lead to feelings of empowerment, and improve self-care behaviors. However, despite these benefits, patient participation in DSMES is low due to barriers such as cost and access. This poster reviews a pilot study to improve diabetes outcomes (HbA1c, physical activity, meal planning, diabetes knowledge), as a result of implementing a DSMES program offered at no cost to all UF Health Jacksonville patients.
