P219 - A Rural Community Coordination Process for Improving Population-level Diabetes Outcomes

People at risk for or living with prediabetes or diabetes face multifaceted challenges within the communities in which they live, work, seek care, learn, and play. In this poster, the authors share insights and outcomes from the Diabetes On Track project, a coordinated community approach using a novel quality improvement process that helps stakeholders use local data to design and implement equitable solutions. The authors describe how stakeholders in public health, DCES, community members, healthcare organizations, elected officials, and human services have worked together within two rural communities to mitigate these challenges and improve population-level diabetes outcomes.
