P235 - Enhancing Diabetes Education with a Produce Prescription: A Multi-Sector Partnership in New Orleans

Food insecurity, or the inability to access enough food for an active, healthy life, is a social driver of health that negatively impacts diabetes self-care and management. Food is Medicine programs, such as produce prescriptions, provide multi-level, multi-sector approaches to addressing food insecurity among people living with diabetes. This poster highlights the New Orleans-based partnership between a federally qualified health care center and a local university to integrate a produce prescription with a diabetes care and education program and how this type of collaboration can be an effective strategy to enhance diabetes education.
