P213 - Reaching Specific Populations with DSMES and DPP Programs Throughout Texas

The Diabetes Prevention and Control Program (DPCP) at the Texas Department of State Health Services (DSHS) collaborates with partners across the state of Texas to implement the National Diabetes Prevention Program (DPP) and Diabetes Self-Management Education and Support (DSMES). This poster discusses how effectively these programs have reached the Hispanic, Non-Hispanic Black, and older (aged 45+) populations within the state and strategies on overcoming barriers, improving program participation, and enhancing sustainability.
