P212 - Community Health Workers: The Unsung Heroes in Diabetes Prevention and Management

Community Health Workers (CHWs) are catalysts in diabetes prevention and management for rural and underserved Northeast Texan residents. Through equitable care, CHWs bridge gaps between individuals of diverse communities and care teams by providing the Diabetes Education Empowerment Program and the CDC National Diabetes Prevention Programs in comfortable and engaging environments. This experience empowers East Texans with confidence in preventing and managing diabetes while improving their quality of life. This poster describes how attendees participate in discussions, practice reflective feedback, and identify resources that play an important role in combating the diabetes epidemic.
