P211 - CHW Navigation: Bridging the Healthcare System for Diabetes Patients Beyond the ER

The Community Health Worker Home Visit Program at Harris Health System addresses the healthcare needs of disengaged individuals with unmanaged diabetes. CHWs serve as bridges between patients and the healthcare system, transforming the way care is delivered. This poster describes a multidisciplinary approach where CHWs meet patients who may have fallen through the cracks of traditional healthcare systems in their homes. By establishing this direct connection, CHWs can facilitate access to primary care services, address the nonmedical drivers of health that often contribute to poor health outcomes, and re-engage patients with the diabetes care team.
