Since the first Food Farmacy at Strawberry Health Center in 2019, Harris Health has been dedicated to addressing nutrition insecurity across our extensive network. This poster outlines how Harris Health in collaboration with our community partners, embeds a population health approach as part of the standard of care for chronic disease management, strategically intervening with people with expressed social needs and an inability to reach their diabetes goals to improve their health outcomes.
