P204 - Assess, Connect, Transform: Mastering the Virtual Art of Addressing SDOH

Social determinants of health impact care and outcomes for people with diabetes. There is a need and process for assessing and addressing SDOH for better outcomes. This poster highlights real-world examples of this in a national virtual environment demonstrating interdisciplinary collaboration and engagement strategies. The poster authors also outline navigating vulnerable conversations, developing assessment tools, integrating assessment into telehealth visits, identifying resources, follow-up, and ongoing needs, and collaborating with health plan partners.
