P612 - Advancing Care for Gestational Diabetes: Insights from an Implementation Science Project

Given the lifelong cardiometabolic risk and potential for developing type 2 diabetes after gestational diabetes (GDM), there is a need for evidence-based interventions that foster health-protective behavior. Yet, the transition from pregnancy to postpartum has been called a missed opportunity. System-level barriers, communication breakdown, and unclear role delineation are some of the challenges. This poster describes a study to apply implementation science strategies to drive the integration of a person-centered, evidence-based care-planning innovation in an urban hospital organized around the ADCES7 Self-Care Behaviors™ framework.
