P608 - Diabetes Management in Primary Care: A Theoretical Framework

The Theoretical Framework for Diabetes Management in Primary Care addresses the complexity of diabetes management between the 37 million affected individuals and their primary care teams. Only 35.8% of people with diabetes are achieving conservative targets. Adapting the Chronic Care Model and integrating social determinants of health with the ORBIT model's optimization process to enhance the individualization of care plans, this framework promotes productive interactions between health care teams and those with diabetes. This poster describes the framework‘s impact on diabetes-related outcomes such as quality measures, burnout, and complication rates while highlighting key concepts and a cyclical approach to optimizing diabetes care.
