P607 - Diabetes Detectives: Discovering Contributing Factors of Severe Hyperglycemia in the Hospital

The "diabetes detectives" approach can be used in any inpatient setting by members of the diabetes care team. This poster describes an approach that uses education and chart detective work (chart audits) to uncover the contributing factors of severe hyperglycemia. As the contributing factors of severe hyperglycemia are uncovered, quality improvement methods can be deployed to remove barriers to safe patient care.
