P604 - Collaborate, Automate, and Iterate: Simplifying Transitions in Care Using EHR Automation

In optimizing diabetes education referrals during care transitions, we fostered collaboration, automation, and continual iteration. This poster describes a collective effort to alleviate the hospital RN burden, ensuring consistent patient education at discharge. Empowering the hospital, we identified crucial survival skills for those with diabetes, guiding them to the right educational resources. Through this journey, we emphasized the iterative process, effective communication, and the vital role of the Plan-Do-Study-Act (PDSA) cycle in ongoing improvement.
