P520 - Use of Continuous Glucose Monitoring in an Underserved Diabetes Population in an FQHC

Diabetes has significant long-term health complications and for many individuals, blood sugars are difficult to manage. Literature shows that CGM benefits individuals with diabetes, with A1C improving as much as 1.6%. However, there remains a knowledge gap in how CGM systems can improve outcomes and glycemic management in underserved and vulnerable populations. This poster reviews a project to add a CGM program at a Federally Qualified Health Center (FQHC) with A1C as the primary outcome.
