P201 - If You Offer It, They Will Come: Diabetes Care and Support in Rural Appalachia

Attending a diabetes support group is part of an integrated approach to diabetes management and a proactive way for those living with diabetes to find information, encouragement, and support. In 2022, Appalachian Regional Healthcare identified a need for diabetes support in rural Appalachia. This poster describes the success of a program that offers an additional layer of diabetes support and has led to new opportunities and most importantly, healthier communities across Appalachia.
