Expanding the role of the CDCES can be a challenge with limited resources across large health systems. The COVID-19 pandemic challenged CDCESs to utilize technology to deliver care in new ways. This forever changed healthcare. This poster showcases how one health system successfully incorporated an inpatient virtual care model to expand patient reach despite decreased CDCES resources. The poster authors share the glycemic care net model they created to help prioritize patients and glycemic management needs.
