P410 - Focusing on Diabetes Care and Education in Primary Care with an Integrated Team Approach

Managing diabetes involves challenges such as behavior change, medication management, and stressors including anxiety and diabetes distress. Most people with type 2 diabetes see a primary care provider for diabetes management; few have access to an embedded Integrated Care Team with a dedicated Diabetes Care and Education Specialist and clinicians from other disciplines. This poster discusses how the CDCES in a primary care setting works with the Integrated Care Team to evaluate barriers related to the multiple demands of diabetes management and how to implement attainable goals for people with diabetes using a shared decision-making approach.
