P417 - Interdisciplinary Collaboration to Improve Diabetes Measures in the Inpatient Setting

Managing inpatients with diabetes admitted with psychosis can be challenging. They often refuse their medications and their A1C levels are not checked within the first 24 hours of admission. As a result, hyperglycemia affects 38% to 47% of this population of patients. Additionally, of the patients who experience hypoglycemic events, only 10% have the event documented. Finally, there have been discrepancies in patient education documentation by nurses. Therefore, collaborating with the interdisciplinary team can help prevent acute complications. This poster reviews the importance of this collaboration and its role in ensuring optimal patient outcomes.
