P403 - Impact of Pharmacists' Assessment of Diabetes Distress in a Veteran Population

Diabetes distress (DD) should be evaluated yearly but is not always integrated into patient care. Pharmacists are well-placed to assist with assessing DD within ambulatory care clinics. This poster reviews a study examining the use of the Diabetes Distress Scale (DDS) within two Patient-Aligned Care Team (PACT) pharmacist clinics at the Veterans Affairs Kansas City Medical Center to see if identifying and providing resources to address DD results in improved glycemic management and lowers DD. The authors share completed intervention group data and information regarding resources offered and barriers to care.
