P401 - Alleviating Diabetes-Related Distress via Personalized Chronic Disease Management

During the COVID-19 pandemic, many patients with chronic diseases were faced with difficult decisions about their care. Routine visits became less accessible, and clinicians noticed that many of their most vulnerable patients were the ones who were the most impacted. In response to these worsening disparities, the Pharm2Home Initiative was created. This poster describes how the initiative aims to improve access to chronic disease care in Solano County, California— where hospitalization rates for diabetes, heart failure, and asthma all exceed the national average. Pharm2Home connects patients from low-income backgrounds to chronic disease management via telehealth and home visits. Visits are conducted by clinical pharmacists and focused on pharmacotherapy management and enhancement. Analysis of this program utilizes a prospective design to examine whether Pharm2Home’s personalized chronic disease management approach induces a change in diabetes-related distress.
