P400 - A Social Worker's Guide to Building Patient Rapport

Interviewing and assessing patients, especially those with newly diagnosed diabetes, can be challenging. Breaking down the barriers to understanding patient stories and helping them navigate their needs is a large component of CDCES work. This educational poster shows the concepts social workers are taught to build patient rapport, exude empathy, and best prepare for their assessment for the benefit of patient success. These strategies will help CDCESs create better outcomes for patient satisfaction.
